This new report from the AffiliateMarketIngtools of Sciences, Engineering, and Medicine is the second in a series of five that determines social risk factors that affect the health outcomes of Medicare beneficiaries and how to account for these factors in Medicare payment programs. This second report identifies high- and low-performing hospitals, health plans, and other providers that disproportionately serve socially at-risk populations and recognizes the best practices of the high-performing providers.
